
Self-Determination Staff
Self-Determination staff support individuals who direct their own services, budgets, and life goals.
This role focuses on helping participants understand their options, make informed decisions, and coordinate supports that align with their personal vision for independence and community living.
What You'll Do in This Role
Working in Self-Determination involves guiding individuals through planning, decision-making, and service coordination. Staff provide support and information while respecting each participant’s independence, ensuring they remain in control of how their services are structured and delivered.
Key Responsibilities
• Assist participants in understanding their service options and available resources • Support development and maintenance of individualized service plans • Help coordinate providers, supports, and community services • Provide guidance with budgeting and use of allocated funds • Encourage informed decision-making and self-advocacy • Communicate with families, vendors, and support teams as needed • Maintain documentation and ensure services align with participant goals
What This Looks Like Day-to-Day
• Meeting with participants to review goals and service plans • Assisting with coordinating services and scheduling providers • Supporting participants in making informed choices about their supports • Communicating with families and service providers • Adjusting plans as needs, goals, or circumstances change
